Question 1

Can vaginismus be treated at home?

Accepted Answer

Yes. With the right structured program designed by a qualified pelvic health physiotherapist, many women can make significant progress with vaginismus at home. Effective treatment combines pelvic floor muscle retraining, progressive desensitisation, and nervous system regulation.

Question 2

Is vaginismus permanent?

Accepted Answer

No. Vaginismus is not permanent. With evidence-based treatment, the vast majority of women achieve full resolution and comfortable, pain-free penetrative sex. It is one of the most treatable pelvic pain conditions when approached correctly.

Question 3

What is the difference between a pelvic floor physiotherapist and a gynecologist?

Accepted Answer

A pelvic floor physiotherapist treats muscular and nervous system conditions of the pelvis including vaginismus, painful sex, vulvodynia, and pelvic floor dysfunction. A gynecologist treats medical and hormonal conditions of the female reproductive system including abnormal bleeding, PCOS, endometriosis, infections, and fertility concerns.

Question 4

Should I see a physiotherapist or gynecologist for painful sex?

Accepted Answer

In most cases, a pelvic floor physiotherapist is the right first step. Dyspareunia is most commonly caused by pelvic floor muscle tension, nervous system sensitisation, or conditions like vaginismus and vulvodynia — all of which respond directly to pelvic floor physiotherapy, including self-guided online rehabilitation programs.

Question 5

What causes endometriosis pelvic pain?

Accepted Answer

Endometriosis causes pelvic pain through chronic inflammation, scar tissue, and adhesions — but also through pelvic floor muscle tension that develops as the body guards against persistent pain. Pelvic floor physiotherapy addresses this muscular layer alongside medical treatment.

Question 6

Can pelvic floor physiotherapy help with endometriosis?

Accepted Answer

Yes. Pelvic floor physiotherapy addresses the muscular guarding, painful sex, and nervous system sensitisation that develop alongside endometriosis. It works best in combination with medical management from a gynecologist.

Question 7

Is vaginal dryness only a menopause problem?

Accepted Answer

No. While menopause is the most common cause, vaginal dryness affects women at many life stages including postpartum, while breastfeeding, and as a result of certain medications. Any significant drop in oestrogen can trigger vaginal dryness regardless of age.

Question 8

Can vaginal dryness cause painful sex?

Accepted Answer

Yes. Vaginal dryness is one of the most common causes of painful intercourse. When vaginal tissues are dry and thin, friction during sex causes burning, tearing sensations, and microtears. Pelvic floor physiotherapy combined with appropriate lubricants and moisturisers directly addresses this pain.

Question 9

What is vulvodynia?

Accepted Answer

Vulvodynia is chronic pain in the vulvar area lasting three months or more with no identifiable cause. It includes burning, stinging, irritation, and rawness that significantly impacts daily life and intimacy. It is treatable through pelvic floor physiotherapy and nervous system regulation.

Question 10

Can vulvodynia be treated at home?

Accepted Answer

Yes. Many women with vulvodynia make significant progress through a structured self-guided online program addressing pelvic floor muscle retraining, nervous system regulation, and progressive desensitisation — all available from home without a clinic visit.

Question 11

Why does sex become painful during menopause?

Accepted Answer

Menopause causes a significant drop in oestrogen, which thins and dries vaginal tissues, reduces natural lubrication, and causes pelvic floor muscles to tighten. Together these changes make sex painful — but they are all directly treatable through pelvic floor physiotherapy, lubricants, moisturisers, and hormonal treatments.

Question 12

What causes painful orgasm in women?

Accepted Answer

Painful orgasm is most commonly caused by pelvic floor muscle tension, endometriosis, or nervous system sensitisation. The pelvic floor muscles contract forcefully during orgasm, and when they are already tight or dysfunctional, this contraction causes pain. It is treatable through pelvic floor physiotherapy.

Question 13

What causes vaginal burning after sex?

Accepted Answer

Vaginal burning after sex is commonly caused by vaginal dryness, pelvic floor muscle tension, vulvodynia, provoked vestibulodynia, reactions to products, or hormonal changes. All of these causes are treatable through the right combination of pelvic floor rehabilitation and appropriate intimate care products.

Question 14

Can stress cause pelvic floor problems?

Accepted Answer

Yes. Chronic stress and anxiety directly cause pelvic floor muscle tension. The pelvic floor holds tension from the nervous system just like the shoulders and jaw — and in women with pelvic pain conditions, this tension is often a significant contributing factor.

Question 15

Is there a connection between jaw tension and pelvic floor tension?

Accepted Answer

Yes. The jaw and pelvic floor are neurologically and fascially connected. When the jaw chronically tightens through stress or clenching, the pelvic floor often tightens in a parallel pattern. Addressing jaw tension is a meaningful part of managing pelvic floor dysfunction.

Question 16

What is provoked vestibulodynia?

Accepted Answer

Provoked vestibulodynia (PVD) is chronic pain at the vaginal entrance triggered by touch or pressure. It is one of the most common causes of painful sex in women and is often misdiagnosed or dismissed for years. It responds well to pelvic floor physiotherapy and self-guided online rehabilitation.

Question 17

Can an online pelvic floor program replace in-clinic physiotherapy?

Accepted Answer

For conditions including vaginismus, dyspareunia, vulvodynia, and pelvic floor dysfunction, a structured evidence-based online program is highly effective. The core treatment tools — pelvic floor retraining, nervous system regulation, progressive desensitisation, and pain education — translate directly to a self-guided format when designed by a qualified pelvic health physiotherapist.

Question 18
